I wish they wouldn't put them in...  I would much rather buy a shoe from 
column one and my pick of insoles from column two.

But since most consumers probably just use their shoes for short walks 
around towns, they don't get to the point where they need a more supportive 
insoles tailored for their feet.  So, while many of us toss them, the stock 
inserts are good enough for most people.

FWIW, I just realized that for the most part, I spend more on my insoles 
than I do on my shoes.  From my perspective, they are well worth the 
investment.

P.S.  As for offering extras, it's not just a few insole designs, many 
(most?) manufacturers have different insoles for every shoe model (based on 
last, perhaps?) and that gets multiplied by each size they carry.  Then, the 
models get updated evey year.  Etc.  There are just too many to keep extras 
on hand.  Plus, I'm sure the manufacturers would prefer you buy another pair 
of shoes, not just the insoles.
