Maybe a little drop of "thread lock" would help out. Try Loctite or
equivalent.

Down the road you might consider a pair of Black Diamond Flick Lock poles.
The adjust via a cam lock instead of the typical twist lock. They are bomb
proof at the cost of a slight weight penalty. They cost about 1/2 the price
of Leki's.
