Wow!  Am I that predictable?  ;-}

I hope you found the tongue depressors. I know I have some with "Montrail"
printed on them. If you can't find them and you can get some neoprene, you
can cut your own. Follow the shape of the tongue of your boot or shoe -
generally wider at the top and narrower toward the toe. Make sure you leave
enough length at the top so the laces don't slip off the top, but not so
long the neoprene hits your ankle.

Can't help much with the rest of your problem though. I leave diagnosis to
the experts. But there are other resources out there for those doing their
own research.

First the disclaimer: I am not in any way associated with the following
resource and though I have been quoted in the book, I have not read the book
nor do I own the book. Also, though I'm a member of the associated yahoo
group, I have probably only ever read a couple of the ezines...

Jon Vonhof has written a book called "Fixing Your Feet" and he also writes a
monthly ezine article with the same name. You can find these resources at:

http://www.fixingyourfeet.com/ (for book info)
http://health.groups.yahoo.com/group/FixingYourFeetEzine/ (for subscription
and archive)

You might find some useful information there.

Hope this helps...
