I'm guessing the larvae is in the grain and somehow survives the processing. I bought a box of raisin bran once (my favorite cereal at the time) and when I opened it 2 or 3 of the moths flew out. The whole top of the contents was covered with grey stringy stuff. I returned it to the store and have never eaten raisin bran again.
