[Cdt-l] Data Book Structure/Format

Thanks to the feedback from Wandering Bob, David Horton, and others I have
added the following to the Data Book:

- distance from point to point
- elevation change from  point to point
- the percent grade of ascents and descents; a grade of  > 10% is
highlighted

I welcome more feedback!
