I found the best solution was a combination of things......I bought REI MTS boxer shorts 
underwear (some kind of wicking fiber boxers) that I wear under my hiking shorts, and carry a 
small tube of neosporin or other antibacterial creme. Has worked well for me over a dozen 
two-week hikes.....if I get a hot spot a little neosporin does the trick unsually by the next 
day....I do usually try to take at least a cat bath day every day...hygeine goes a long way 
toward avoiding rash...
