I suspect you are very right.  I was a private pilot for years and the vast
majority of accidents were exactly that, a series of small decisions that
compounded in a very bad way.

I think I coined the term "incremental stupidity" a few years ago on the
list.? I've fallen to this many times in the past and hope to avoid doing so
again.? In fact, right now, I'm probably doing it!

It seems like many unfortunate situations aren't the result of one
monumentally stupid decision or catastrophic gear failure but instead are
the result of a series of small mistakes that lead up to the proverbial
"straw that broke the camel's back"
