Please be warned not to take this as gospel. There ARE errors, I have found two, so no doubt there are more. I will update as I find them. The same link should continue to work. 

Thank you to the folks who gave feedback, very helpful. 

Someone asked about a print formatted version. I think label format could be generated in Word using print merge, but I haven't used that since the late 1990s.
