Whew.  Lets have another LNT lesson:

NO!

Get some water, wash your salty bod *away* from the stream!  *Then* if you 
want to get your clean self in the stream, go for it!

Totally disagree with the folks calling for ditching the underwear
altogether

No underwear = serious ball chaff in my world.  I guess it just depends on
your personal erruhm... equipment, but I think you run the risk of shifting
the problem to another region

I recommend non cotton, non boxers underwear.
I remember a guy on the AT who used a g string.  I cannot speak to that but
I always wear boxer briefs.  (which I don't use when not hiking.  I much
prefer boxers with more breathing room, but when hiking there is just too
much friction to let things be dangling around in boxers or commando)

as for treatment

liberal application of talcum power to keep it all dry in the sleeping bag
at night, gold bond cream in the day, and have dedicated clean (er)
sleeping garments like some very thin silk ones.  keeps the bag cleaner,
keeps you cleaner and drier.
